Do I Need A Referral?

A.

Generally we do advise our patients to see a physician prior to starting treatment with us. Most insurance companies require a prescription to certify the medical necessity of physical therapy. To find out if you need a referral for physical therapy, call the number on your insurance card and ask if you need a PCP referral for outpatient physical therapy services. If a referral is required, it will need to be in place before your initial evaluation with us.

What Insurances Do You Accept?

Family Physical Therapy accepts Anthem Blue Cross/Blue Shield, CIGNA, Harvard Pilgrim, and most other group health payers. We also take Medicare, claims for worker's compensation, motor vehicle accidents and personal injury.

How Much Will My Insurance Cover?

A.

Each insurance policy is different and each treatment program is different. However, we are happy to verify your insurance coverage and benefits prior to your therapy. We will advise you with the most accurate information and approximations available.

If I Don’t Have Insurance, How Much Do You Charge?

A.

We do accept patients on a cash-paying basis. Your rate will depend on your specific program.

How Soon Can I Start?

A.

We can generally offer an appointment within 24-48 hours. We try to accommodate your schedule whenever possible.

What Should I Bring?

A.

You will need to bring your insurance card. If it is a workers' compensation, motor vehicle, or personal injury, bring your your claim number, and contact information, including name of rep, address and telephone number. You may also be required to bring your referral/prescription for physical therapy from your physician. Your therapist must have this in order to perform your evaluation.

What Clothes Do I Need To Wear During Therapy?

Please wear loose, comfortable clothing and shoes - tank tops or shorts for arm or leg injuries. If necessary, we will provide you with a gown for use in our private treatment rooms.

What Should I Expect On My First Visit?

A.

Your first appointment will be an evaluation with the physical therapist to determine your plan of care. You and your therapist will determine how often treatment is needed and will then create a plan of care that suits your needs.

How Long Do I Need To Attend?

A.

Based on your diagnosis, your therapist will decide what course of treatment is needed and how long it will be necessary for you to attend.

How Long Will My Visit Last?

A.

The initial visit is generally about an hour. We will ask you to complete our patient forms before meeting with your therapist for your evaluation. We will also initiate treatment on your first visit. Subsequent visits usually last between 30 minutes and one hour, depending on your program.

Do You Treat Minors?

A.

Yes. We treat patients of all ages. For patients under the age of 18 years of age, we will require parents/ legal guardians to sign an authorization form.
